Cultural Competency in Birth Work

Chanel Blackmore, Black Pearl Doula Collective September 30, 2026

Cultural competency in birth work means understanding and honoring how a family's culture, traditions, and history shape their pregnancy and birth experience, and providing support that respects those values. Black Pearl's training centers culturally aligned care for Black families and communities.

What cultural competency is

Culturally competent care is not about memorizing traditions. It is about listening, asking, and honoring what matters to each family: their language, their values, their community, and the practices that give them comfort and meaning. It also means understanding how bias and history shape maternal healthcare.

Why it matters for Black maternal health

The CDC reported that in 2024, Black women experienced 44.8 maternal deaths per 100,000 live births, more than three times the rate for White women. Culturally aligned support, advocacy, and education are part of the response. Doulas who understand Black families' experiences can help them feel respected, informed, and supported.

Ancestral and traditional practices

Black Pearl honors ancestral wisdom, Indigenous practices, and diasporic traditions as sources of cultural connection, comfort, and community. Traditional practices such as herbal remedies are discussed as cultural and wellness traditions, not as medical treatment. Doulas never present traditional practices as cures, and they encourage clients to discuss health questions with licensed providers.

How training builds cultural competency

Black Pearl's certification program includes cultural competency and anti-bias education as core curriculum, not an afterthought. Trainees learn how to provide culturally aligned support, advocate respectfully within medical systems, and serve families across traditions and preferences.

The non-clinical scope

Cultural support stays within the doula's non-clinical scope. Doulas provide education, comfort, and advocacy; they do not diagnose, treat, or make medical decisions. Medical questions always belong with licensed healthcare providers.
